1. Go to the Membership Registration
2. You will need the following information to accomplish your Online Membership Registration:
- Last Name
- First Name
- Middle Name
- Date of Birth
3. Fill-out the application form (
Take note that the form consists of several tabs, such as Member Info, Other Info, Address, Contacts, Heirs, Member Category, and Employment History, so you should go through each tab and input the details being asked.)4. Submit your application. You'll be redirected to the Successful Registration page and you'll also receive an SMS confirmation.
