The  PhilHealth Identification Number (PIN) is used for identification, and verification if you and/or dependents are eligible for a PhilHealth Benefits and privileges provided by the government's National Health Insurance Program.

2 Member categories of PhilHealth

  1. Direct Contributors - These included employees, self-employed professionals, and OFWs who can pay PhilHealth contributions. 
  2. Indirect Contributors - These include Senior Citizens, indigents, and other members with government subsidy. 

All members can enjoy subsidy for the following healthcare costs: 
  • Hospitalization
  • Outpatient Services such as hemodialysis, laboratory tests, day surgery, etc
  • Treatment for certain Cancers and other diseases
  • Pre and post Natal delivery, child delivery
  • Covid-19 testing and hospitalization

PhilHealth ID Requirements for Membership for Employed and Self-Employed: 
  • Two accomplished copies of the PhilHealth Member Registration Form (PMRF)[1]  pmrf_012020.pdf (philhealth.gov.ph)
  • Two latest 1×1 ID pictures
  • At least two valid IDs or a photocopy of your PSA-issued Birth Certificate

Registration procedures 

For Newly Hired And Existing Employees Without PIN YetFor Newly Hired Employees With PIN
  • Report your PIN to your employer for them to indicate the same in their ER2
Premium Requirements
  • Premium contributions are shared by the employee and the employer, the amount of which is determined based on the new premium contribution rate. After deducting half of the premium requirement from your monthly salary, total premiums are remitted by your employer to PhilHealth.

New Application Procedure

  1.  For PhilHealth Online Registration - go to eregister.philhealth.gov.ph or www.philhealth.gov.ph and click online services then register under electronic registration then click the Proceed Button, Accept Button and Agree to the terms and conditions. 
  2.  Accomplish the required personal and contact information 
  3.  You have a choice to upload or email the scanned copy of your Valid ID. 
  4.  Enter the CAPTCHA code and click Submit Registration
  5. Your Transaction Number and instruction on the next step will be sent via Email. Make sure to check your Inbox from PhilHealth Member Information system, if you can't see it check the junk or spam folder.
  6.  Once received click the hyperlink to activate your Philhealth Number
  7. Within three (3) weeks you will receive another email containing your PhilHealth Identification Number (PIN). 

In case you want to amend your data

1. Download PhilHealth Member Registration Form or (PMRF)

2. Tick FOR UPDATING on the upper right-hand corner of the PMRF

3. Fill out PMRF as appropriate

4. Submit properly filled out PMRF to the nearest PhilHealth Office

5. Await printout of updated Member Data Record


Continuing your coverage
If leaving formal employment to venture into self-employment
  • Request for amendment of membership to become a Self-Earning Individual, declare your monthly income, and pay the required premium at any PhilHealth Office or accredited payment center or through the PhilHealth Member Portal
  • The premium contribution shall be based on the declared income.
  • Payment can be made monthly, quarterly, semi-annually or annually.
If seeking employment as land-based overseas Filipino worker

Indicate existing PhilHealth Identification Number (PIN) when processing registration and payment to PhilHealth as a migrant worker.

If already at age of retirement, have contributed at least 120 monthly premiums, and is not gainfully employed:

Who are the Formal Economy members?
• Government Employee

An employee of the government, who renders services in any of the government branches, military or police force, political subdivisions, agencies or instrumentalities, including government-owned and-controlled corporations, financial institutions with original charter, Constitutional Commissions, and is occupying either an elective or appointive position, regardless of status of appointment.

• Private Employee

i. Corporations, partnerships, or single proprietorships, NGOs, cooperatives, non-profit organizations, social, civic, or professional or charitable institutions, organized and based in the Philippines including those foreign owned;

ii. Foreign governments or international organizations with quasi-state status based in the Philippines which entered into an agreement with the Corporation to cover their Filipino employees in PhilHealth;

iii. Foreign business organizations based abroad with agreement with the Corporation to cover their Filipino employees in PhilHealth.

• All other workers rendering services, whether in government or private offices, such as project-based contractors, and the like• Owners of Micro Enterprises• Owners of Small, Medium and Large Enterprises• Household Help

as defined in Republic Act 10361 or “Kasambahay Law”

• Family Drivers
